I was thinking about something like this the other day. Metal processing requires heating until it's molten (obviously). The furnaces are so big that they have giant crucibles and channels to tap the molten material. There are usually walkways above all the machinery but there's no way to see inside the furnace let alone somehow get inside it. It may be possible to fall into a crucible, some part of the forge or some such machine. Maybe there's an accident which causes spillage. It's said the worst way to die is being set on fire. Drowning in concrete also sounds awful. So you combine them to get drowning in molten steel. I imagine that would be the worst way to die.
