They still don't pass as women now, they just take hormones that "soften" their facial features, give them man titties, and keeps their hairline at a norwood III instead of regressing to VI, and if they have the money then they'll get plastic surgery to make their faces look more feminine + lip fillers.

TL;DR they had less resources back then and used what they could/what was available to them at the time
